Our customer is a leading coal distribution company - an integral part of Australia’s coal supply chain. With facilities including rail, coal storage, ship loading and associated infrastructure, the organisation ensures a reliable coal supply from Australia to the world export market.

The challenge

Hazards are inevitable in the mining industry, and they can be challenging to track and mitigate. The company's policies and procedures are industry-leading. They monitor, track, and reduce hazards and continually improve their Sustain Management System to ensure employee and environmental safety. 

However, this is a complex process that requires collecting many documents and files. A single incident can involve a dozen people, pictures, documents, reports and various actions across departments, inside and outside the organisation. A purpose-built solution was needed to improve this important process.

The solution

After finding that no off-the-shelf product could offer the flexibility and control they required, the company partnered with PhoenixDX to build a purpose-built Incident Management System as part of their Sustain Management System.

The solution centralises the reporting, investigation, and management of all incidents, hazards, actions, and events. Automated workflows guide relevant team members - creators, moderators, investigators, owners, and reviewers -through a 17-step process from creation to completion, with notifications keeping everyone on track. Batch status updates dramatically reduce investigation time, and once all sections are complete, the system automatically generates a comprehensive incident report.

Role-based permissions ensure appropriate access to sensitive information, while a centralised dashboard gives management full visibility across the entire process. Critically, all employees and contractors can lodge incidents, near misses, or hazards directly, without needing to speak to a manager first. This removes potential barriers to reporting, improves transparency, and empowers everyone to play an active role in workplace safety.
